How To Obtain A Card

In order for you to receive an application form, you can download it right off of our webpage or contact the office at phone (613)687-4141 or fax (613) 687-1515 in and we will mail one to you, or you can drop in at the office to pick one up.

On the application form it asks for 3 members of your family that may be a member of our community, you do not need 3.

You must provide us with the following information:

-band #, long birth certificate or baptismal certificate (records showing who your parents are)
-oral or written statements by an Elder known to Bonnechere Algonquin Community
-sworn affidavits
-other information showing Aboriginal Ancestry
-try to show part of your family tree.
